Counselor Application Form

The CMT counselor and CIT application form is available in PDF format. To open this form, you must have the Adobe Acrobat Reader installed on your computer. To use this form:

• Click on the link for the form, above. The form should open in your browser window.
• Fill out the form directly on your computer. You can type into all of the fields of this form while it is open on your computer. Use your mouse or the "Tab" key to move from field to field. Use the "Clear Fields" button at the top of the form to erase everything you have typed and start over. To "check" a box for a yes-or-no question, just click in the box with your mouse.
