Camper Registration and Medical Release Forms

CMT registration and medical forms are available in PDF format using the links below:

• Camper Registration Form – You can use one registration form for up to three children. All children must be from the same family.
• Medical Release Form – You must complete one medical form for each child you are registering.

To open these forms, you must have the Adobe Acrobat Reader installed on your computer.

Instructions:

• Click on the link for the form, above. The form should open in your browser window.
• Fill out the form directly on your computer. You can type into all of the fields of this form while it is open on your computer. Use your mouse or the "Tab" key to move from field to field. Use the "Clear Fields" button at the top of the form to erase everything you have typed and start over. To "check" a box for a yes-or-no question, just click in the box with your mouse.
• Print the form and mail it or drop it by the church office:

• We are expecting four full camp sessions again this summer. Campers are registered on a first come, first served basis. Early registration is encouraged!
